Creation is as simple as:

1. If you encounter a problem, try to solve it.
2. If you cannot solve it, mark/document/publish it and try to
work around it for now.
3. If you cannot work around it, leave it to other people and
offer help.

Which is pretty much what this list here is for ;-)
